This form should not be used to access urgent help and may take up to 5 working days to be processed.
You can use this service if you:
- are registered at the surgery
- have already seen a doctor, nurse or other healthcare professional and have been invited to do so
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
Call 999 or go to A&E now if:
- someone’s life is at risk – for example they have seriously injured themselves or taken an overdose
- you do not feel you can keep yourself or someone else safe
A mental health emergency should be taken as seriously as a physical one. You will not be wasting anyone’s time.
If we need to contact you regarding the submission of your form, we may respond directly through the website and aim to do this within 5 working days of receipt of your form. You will be notified of any responses via email. Please remember to check your email account’s spam/junk folder
