Health Visitors
Health visitors are registered nurses and midwives with an additional community public health qualification. They offer support and practical advice on many aspects of child development as well as family health.
If you have a child aged 0-5 and you live in Bracknell, Wokingham, Reading or West Berkshire, it’s really easy to get in touch with the Health Visiting team using the ChatHealth service. You don’t need to give your name and you will be able to get confidential advice and information from us direct to your phone, about any concerns you may have about the health of your baby or child.

Community (District) Nurses
The nurses work closely with all members of the primary health care team. The service aims to provide holistic nursing care for patients who are usually housebound. Included in the team is a community matron who can see patients with more complicated problems, and helps to manage them in their own home. Community nurses can refer on to other agencies such as Social Services. The community nurses can be contacted directly on 01635 292080.
Midwife
The community midwife from the “Rose Team” runs a weekly clinic at Chieveley Surgery, caring for expectant mothers from 8 weeks to post-birth. Contact numbers for your chosen hospital can be found on your antenatal notes.
Talking Therapies
Problems such as anxiety, depression, stress and phobias are very common. Talking Therapies is a friendly and approachable service that helps people with these problems. You can refer yourself to Talking Therapies or visit your GP for a referral.
