Vaccination Consent and Vaccination Information
In line with NHS England requirements, patients may be asked some additional questions when booking their vaccination appointment. This helps us confirm consent, eligibility and ensure you are offered the most appropriate vaccine.
For patients who routinely receive text messages from the practice, a questionnaire will be sent by text message with a link to complete an online consent form for your vaccination.
If you are unable to complete the online form, a member of our team will contact you and go through the questions and consent process with you over the phone instead.
