Dear Patient
We are delighted that you will be attending our COVID-19 Vaccination Clinics, please make sure you have reminders set for both your first dose and second dose appointments. Please bring this letter with you to the appointment and complete the name and contact details of your next of kin at the foot of this letter.

CLINIC SAFETY INFORMATION
- You must not attend if you or any of your household have ANY of the covid-19 symptoms (high temperature OR cough OR loss of OR change in taste OR smell), or if you’ve been told to isolate by track & trace – please call us to cancel so we can offer your slot to someone else.
- Please arrive on time and come alone if possible. Carers/drivers should wait in the car unless you need support to get to and from the building.
- Please wear a face covering / mask, apply hand gel on entry, keep to one way system and maintain social distancing at all times.
- Wear clothing that allows for easy access to your upper arm.
- Do not bring unnecessary bags or belongings to the clinic.
- There will only be limited toilet facilities on site.
CLINIC INFORMATION
Parking
Please note that access to the racecourse is via the ‘new bridge’ at the Thatcham end of Hambridge Road. It is best accessed via the A4 from the cross road traffic lights. Turn into Hambridge Road and at the roundabout go straight across over the new bridge signposted for the racecourse. Follow the signage for the COVID clinic. Disabled parking spaces will be available for less mobile patients.
How long will I wait?
We will be working as quickly as we possibly can, being mindful of social distancing and vaccine requirements. We hope to keep queues to a minimum but please do come prepared for inclement weather.
To avoid long queues, you should aim to arrive at the building entrance no more than five minutes before your appointment time. When in the queue, please maintain social distancing.
Whilst we aim to keep to schedule, we will give priority to those with a disability and invite them in on arrival (subject to capacity). This may mean a slight delay for some.
Duration
We aim to get you through the vaccination process within 10 minutes. Following this you will be required to wait in an observation area for 15 minutes.
Will I be safe?
We are following all government advice to keep our patients and ourselves safe. Staff will be wearing personal protective equipment (PPE) in line with Public Health England guidance. We will be operating a one-way system through the building. Hand gel will be provided and staff will be available to provide guidance and assistance.
I have a carer for someone in this cohort, can they have the vaccine?
Unfortunately not. Carers will be called at a later date.
I am a health and social care professional, what do I need to bring?
If you are a health and social care professional you MUST bring your work ID. This could be a work ID card or payslip with salary redacted. We will need this to verify your identity and eligibility for the vaccine.
LAWFUL DATA SHARING
I understand that by attending the COVID-19 Vaccine Clinic my data will be shared with my GP Practice, NHS England and the NHS Business Services Authority.
The legal basis for sharing this information is: Your name, address and information about your COVID-19 vaccination will be sent to your GP practice so they can update your health record.
