We are in the process of streamlining our processes for monitoring blood pressure. As such, we are phasing out paper blood pressure forms and diaries, where possible. If you are required to monitor your blood pressure at home, you will be requested to do so by a clinician.
Your clinician will send you a text with a daily reminder and link to submit a reading each day for 4 or 7 days. You will need an internet enabled mobile phone to be able to do this.
If you don’t have a smartphone, your clinician will instead ask you to complete a home BP monitoring paper diary and submit it to the practice.
If you wish to continue monitoring your blood pressure at home, you are welcome to do so. If you have concerns around your blood pressure and need clinical advice, please call the surgery to arrange an appointment.
