All requests for DVLA medicals that require to have the visual acuity testing undertaken, then this need to be completed on the D4 form or taxi medical form by the optician before applying to the Downland Practice to have the rest of the medical undertaken. If applications for DVLA medicals are received that require visual acuity testing and the DVLA form has not been complete by the optician then the paperwork will be returned to you for the appropriate action.
