New Patient Registration
Thank you for choosing The Downland Practice to be your registered practice.
If you are residing in our catchment area (permanently or temporarily) and would like to register with the practice, please visit Chieveley Surgery with at least one form of photo ID and at least one form of proof of current address. Our team will ask you to fill out a registration form and take copies of your ID and proof of address. These copies will be processed with your registration form and then securely disposed of; copies will not be retained on our records. To minimise time spent in the practice during the COVID-19 pandemic, where possible please download and complete the forms prior to visiting the practice. Please complete one registration (GMS1) form per person (including children). The health questionnaire should only be completed by patients over 16 years of age.
Please ensure that you are a resident in our catchment area and living at the address provided to us before registering as a new patient with the practice. We will be unable to accept registration of any patients residing outside of our catchment area. To find your nearest GP surgery, please visit www.nhs.uk/service-search/find-a-gp.
