New Patient Registration

Thank you for choosing The Downland Practice to be your registered practice.

If you are residing in our catchment area and would like to register with the practice, please print and complete the below forms and bring them to Chieveley Surgery. Alternatively, please collect forms from our team. Please ensure both forms (the GMS1 form and the adult health questionnaire) are completed for each patient. The health questionnaire should only be completed by patients over 16 years of age. If you would like online access and have completed the request form within the adult health questionnaire, you will need to show a form of photo ID at the point of registration.

Please ensure that you are a resident in our catchment area and living at the address provided to us before registering as a new patient with the practice. You can do this by checking our boundary map. We are unable to accept registration of any patients residing outside of our catchment area. To find your nearest GP surgery, please visit www.nhs.uk/service-search/find-a-gp.

If you have any further questions regarding the registration process, please ask a care coordinator.

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
